Clinical Practice Pharmacist

About the role

A vacancy exists for a Clinical Practice Pharmacist based at Life Brenthurst Hospital, reporting to the Pharmacy Manager. The successful candidate will deliver a patient‑centred clinical pharmacy service as part of the healthcare team, ensuring safety, efficacy and sustainability in medication use.

Key responsibilities

• Implement all components of the LHC antimicrobial stewardship programme, including compiling and updating antibiograms.
• Monitor unit‑level microorganisms, discuss trends with microbiologists and drive improvements in programme metrics.
• Deliver key performance indicators in clinical pharmacy processes to improve patient safety and outcomes.
• Utilise surveillance systems to prioritise work in wards and support cost‑of‑care initiatives.
• Provide drug information, evidence‑based guidance and training to healthcare professionals.
• Enhance medication safety management and intervene in medication errors at ward level.
• Comply with legal, professional and procedural requirements for dispensing prescriptions.
• Participate in internal stakeholder meetings, industry conferences and external stakeholder engagements.

Required profile

• B Pharm degree with a certificate in ward pharmacy/clinical skills (or studying towards one).
• Minimum of 2 years of relevant hospital experience.
• Current registration with the South African Pharmacy Council as a pharmacist.
• Understanding of pharmacy legislation, regulation and industry challenges.
• Proficiency in Microsoft Office applications.
