Clinical Practitioner – Managed Service Network for Functional Conditions

About the role

We are establishing a Managed Service Network (MSN) to support adults with functional conditions in Grampian. The Clinical Practitioner will join a small multidisciplinary team to deliver patient‑centred, goal‑directed therapy and promote self‑management.

Key responsibilities

• Provide clinical assessment and collaborative formulation for adults experiencing functional conditions.
• Offer practical support and guidance to help patients manage symptoms, navigate services and improve daily functioning.
• Develop and implement care pathways, education materials and self‑management resources.
• Work within the MSN framework to integrate services across the health system.
• Participate in publicity activities as required by the NHS Grampian Charity funding.

Required profile

• Qualified clinical professional from any background willing to work with functional disorders.
• Strong motivation to develop innovative, person‑centred services.
• Enthusiasm for collaborative work and system‑wide improvement.
• Agreement from current line manager if applying for a secondment.
