Registered Nurse – Community Care at Home Unit

About the role

The Registered Nurse will lead the Community Care @ Home unit, providing clinical care, conducting home visits, and overseeing a panel of home‑based carers. Reporting to the Unit Manager, the RN ensures high‑quality patient support both in private homes and institutional settings.

Key responsibilities

• Deliver clinical care to patients as required.
• Conduct home visits, audit client files, and report health status to doctors.
• Select, orient, train, and develop home‑based carers.
• Manage carer performance, discipline, and contractual administration.
• Develop marketing strategies and partnerships for the unit.
• Oversee administrative processes, including timesheet verification and payments.
• Complete quality assessments, letters of motivation, and feedback actions.
• Perform detailed patient assessments at homes or medical facilities.
• Assess patients prior to admission to the Sub‑Acute facility.

Required profile

• Registered Nurse with current SANC registration.
• 10–15 years of clinical practice, including private community health experience.
• Valid driver’s licence, own vehicle, and good knowledge of Cape Town.
• Strong written and verbal communication skills.
• Ability to work independently in high‑pressure environments.
• At least 5 years of staff management and experience with disciplinary procedures.
• Team‑player attitude and reliable attendance for periodic calls.

Required skills

• BLS (Basic Life Support) certification.
• Knowledge of ICD‑10 coding.
• Current SANC registration.
• Proficiency with standard computer applications and mobile apps.
