Licensed Practical Nurse (LPN) – Direct Care & Supervision

About the role

The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a long‑term care facility while supervising nursing assistants. The role ensures compliance with federal, state and local regulations and maintains the highest quality of care at all times.

Key responsibilities

• Deliver direct nursing care to residents, including assessments, medication administration, and wound care.
• Supervise and coordinate the daily activities of nursing assistants, ensuring adherence to policies and procedures.
• Maintain and update nursing service manuals, policies, and resident discharge plans.
• Prepare and submit reports, incident forms, charge slips, and daily census data.
• Participate in quality assurance programs, facility surveys, and inspections.
• Collaborate with other resident services to coordinate comprehensive care plans.
• Handle admissions, transfers, and discharges of residents as required.

Required profile

• Current LPN license in good standing.
• Knowledge of federal, state and local regulations governing long‑term care facilities.
• Experience supervising nursing assistants or similar staff.
• Strong organizational and documentation skills.
• Ability to work collaboratively with multidisciplinary teams.
