Registered Nurse – Clinical Intake Officer

About the role

We are seeking an experienced Registered Nurse to join a U.S.-based healthcare organisation’s Clinical Intake team on a remote basis from South Africa. The role focuses on reviewing referrals, determining eligibility for home health services, and guiding patients to appropriate levels of care without providing direct bedside care.

Key responsibilities

• Review incoming patient referrals from electronic platforms, hospitals, physicians and other facilities.
• Assess clinical documentation to confirm eligibility for home healthcare services.
• Evaluate patients against U.S. home health regulations, including homebound status and skilled nursing or therapy needs.
• Determine acceptance based on clinical guidelines and insurance requirements.
• Identify candidates for palliative or hospice services and make internal referrals.
• Manage each referral from intake through discharge, maintaining accurate records.
• Collaborate with case managers, care coordinators, physicians and internal clinical teams to obtain additional information.
• Apply sound clinical judgement while following compliance standards and escalating complex cases as needed.

Required profile

• Current Registered Nurse qualification.
• Several years of nursing experience, preferably with adult and geriatric populations.
• Experience in home healthcare, case management, utilization review, discharge planning, hospice, palliative care, oncology or medical‑surgical settings is highly advantageous.
• Ability to interpret clinical records, diagnoses, hospital documentation and care plans.
• Strong critical‑thinking and problem‑solving abilities.
