Regional Care Coordinator

About the role

The Regional Care Coordinator will be based at Life Healthcare's Head Office and report to the National Case Manager. This role drives patient‑centric, value‑based care across the region, ensuring care pathways improve clinical outcomes, patient experience and system efficiency.

Key responsibilities

• Lead and mentor a team of Care Coordinators to deliver high‑quality, patient‑focused services.
• Conduct weekly feedback meetings, track KPIs and enforce best‑practice playbooks.
• Identify and remove operational impediments, escalating when necessary.
• Ensure alignment with corporate policies, standards and execution quality.
• Flag digital capability issues and work with product owners to resolve them.
• Maintain recruitment and succession plans; onboard and train new coordinators.
• Drive continuous professional development and succession planning within the region.

Required profile

• Proven experience in coordinating healthcare services or similar patient‑care programs.
• Strong leadership and coaching abilities.
• Excellent communication and stakeholder‑management skills.
• Ability to analyse performance data and implement improvement actions.
• Commitment to delivering value‑based care and adhering to best‑practice standards.
