Permanent Social Worker

About the role

A permanent Social Worker is needed for Life St. Dominic’s Rehabilitation Unit, reporting to the Therapy Services Coordinator. The role focuses on neurological rehabilitation within a growing network of rehabilitation units.

Key responsibilities

• Evaluation, programme planning and treatment of patients admitted to the unit.
• Education of patients and their families about the patient’s condition.
• Setting common psychosocial goals with patients and the multidisciplinary team.
• Co‑ordination of family meetings, counselling programmes and discharge planning, including placement and return‑to‑work planning.
• Active participation in continuous professional development activities of the unit.
• Building relationships with referring doctors and therapists.

Required profile

• Degree in Social Work with a special interest in neurological rehabilitation.
• Experience in neurological‑related counselling and discharge planning (advantage).
• Ability to work effectively in a team environment.
• Commitment to quality patient care and customer satisfaction.
• Willingness to work some weekends and public holidays.
