Hospital Social Worker

About the role

We are seeking a dedicated Hospital Social Worker to join our adult social care team. Working on the Discharge to Assess (D2A) pathway, you will help ensure safe, timely, and cost‑effective care transitions for patients.

Key responsibilities

• Conduct comprehensive assessments and develop person‑centred care plans.
• Navigate funding streams and statutory duties to secure appropriate resources.
• Collaborate with multi‑disciplinary teams, including clinicians and community partners.
• Support patients through the D2A discharge process, ensuring continuity of care.
• Maintain accurate records and adhere to legal and organisational frameworks.

Required profile

• Proven experience in strengths‑based social work practice.
• Ability to work effectively in a fast‑paced, pressured environment.
• Strong understanding of adult social care funding and statutory responsibilities.
• Excellent communication and teamwork skills.
