Primary Care Lead – Stroke Managed Clinical Network

About the role

NHS Grampian is looking for a Primary Care Lead to join the Grampian Stroke Managed Clinical Network (MCN). The role is part‑time, with flexible working patterns totalling around 10‑12 hours per month, and focuses on improving stroke care across primary‑care settings in the region.

Key responsibilities

• Support the implementation of evidence‑based practice within primary‑care stroke services.
• Monitor and report on clinical standards to ensure high‑quality care.
• Develop and promote stroke care pathways that provide equitable access across Grampian.
• Identify and raise pathway issues raised by primary‑care colleagues.
• Contribute to strategic planning for stroke prevention, rehabilitation pathways, and TIA management.
• Collaborate closely with the Stroke MCN Clinical Lead and the wider stroke core team.

Required profile

• Demonstrated knowledge and experience in the provision and development of stroke services.
• Strong understanding of primary‑care services within Grampian.
• Ability to work across organisations and with multiple partners to drive change.
• Motivated, engaging, and committed to improving stroke care outcomes.

Required skills
