Clinical Nurse Coordinator – Remote

About the role

We are looking for a clinically‑oriented Nurse Coordinator to provide remote patient services and ensure accurate clinical coordination and documentation. The role supports patients, physicians, and internal teams while complying with state and Medicaid requirements.

Key responsibilities

• Coordinate and schedule biannual nursing assessments for patients.
• Communicate clinical needs and patient concerns to the Director and Patient Services team.
• Prepare and send required clinical documentation to physicians, including faxed documents.
• Inform physicians of approved hours and services for Medicaid compliance.
• Prepare and submit state reports when patients are admitted to or visit a hospital.
• Complete and maintain incident reports accurately and promptly.
• Ensure clinical records are complete, accurate, and up‑to‑date.
• Liaise professionally with physicians, patients, families, and internal clinical teams.
• Escalate clinical concerns to the Director or relevant personnel as needed.

Required profile

• Nursing qualification or relevant clinical background (preferred).
• Previous clinical experience (strongly preferred).
• Strong understanding of clinical documentation and patient‑care processes.
• Excellent written and verbal English communication skills.
• Ability to handle confidential patient information with care.
• High attention to detail and accuracy.
• Good organisational and administrative abilities.
• Capability to work independently in a remote environment.
