Primary Care Ambulatory Registered Nurse

About the role

The Primary Care Ambulatory Registered Nurse partners with physicians and advanced practice providers to deliver safe, high‑quality primary care in an outpatient setting. The role focuses on patient safety, continuity of care, telephone triage, and disease management across a diverse patient population.

Key responsibilities

• Assess and monitor patients with acute and chronic conditions such as diabetes, hypertension, and respiratory disorders.
• Develop, implement, and evaluate evidence‑based care plans in collaboration with clinicians.
• Coordinate care across the outpatient continuum, including referrals to specialists and ancillary services.
• Provide comprehensive patient and family education on disease processes, medication regimens, and preventive health.
• Manage and triage incoming patient communications, directing patients to appropriate levels of care.
• Conduct independent nurse visits, including wound checks, IV fluid administration, and medication delivery.
• Support medication management and safety in partnership with the care team.

Required profile

• Current Registered Nurse (RN) license.
• Strong clinical judgment and critical‑thinking abilities.
• Experience in primary‑care or ambulatory nursing preferred.
• Ability to work collaboratively with physicians, advanced practice providers, and multidisciplinary teams.

Required skills

• Proficiency in patient assessment and documentation.
• Knowledge of medication administration, including antihypertensives, anticoagulants, anti‑emetics, NSAIDs, and diuretics.
• Familiarity with telephone triage protocols and care coordination workflows.
