Renal Clinical Care Coordinator

About the role

The Renal Clinical Care Coordinator will be based at Life Springs Parkland Hospital on a six‑month fixed‑term contract. Reporting to the Patient Service Manager, the role bridges gaps in the renal patient journey, supporting the Renal Unit Manager and the multidisciplinary team to deliver holistic disease management.

Key responsibilities

• Identify and manage deviations from the clinical pathway, including pre‑assessment, authorisation and re‑authorisation.
• Oversee financial risk linked to the pathway, support bill auditors and resolve funding disputes.
• Monitor, analyse and report on pathway costs and trends, presenting findings to responsible parties.
• Act as the primary point of contact for patients from diagnosis through self‑directed care, providing education and resources.
• Receive and respond to criteria‑based referrals within 8 working hours, ensuring diagnostics and follow‑up appointments are booked.
• Track test results, liaise with doctors and MDT members, and coordinate MDT meetings (preparing notes, images and recording outcomes).
• Update patient records with MDT outcomes and communicate actions to internal and external healthcare partners.
• Assess patient needs for support services and facilitate appropriate referrals.

Required profile

• Experience in renal care coordination or related health‑administration roles.
• Understanding of clinical pathways, pre‑assessment, authorisation processes and clinical coding.
• Ability to manage financial and clinical risk and support billing audits.
• Strong organisational and communication skills to liaise with multidisciplinary teams and external partners.
