Clinical Case Manager (Nurse)

About the role

Talent Match Consulting is seeking a dedicated Clinical Case Manager to join a hospital team in Gauteng. The role focuses on delivering safe, cost‑effective patient care while supporting the hospital’s commitment to community healing.

Key responsibilities

• Provide patient care in line with the South African Nursing Council scope of practice.
• Coordinate holistic care plans and supervise their implementation.
• Attend shift handovers and communicate clearly with patients about their treatment.
• Promote health and wellbeing through patient education.
• Audit health records to ensure regulatory compliance.
• Manage nursing stock, ensuring accurate charging and crediting.

Required profile

• Current registration with the South African Nursing Council.
• Relevant nursing qualification and at least 2 years experience as a hospital case manager.
• Advanced computer literacy and flexibility to work varied shifts.
• Ability to uphold health standards and legislation.
• BLS/ALS certification is an advantage.
