Registered Nurse Case Manager – Hospital Group

About the role

The client is looking for an experienced and compassionate Registered Nurse to act as a Case Manager within a hospital group. The role involves coordinating patients’ treatment journeys from admission through discharge and after‑care, ensuring seamless clinical documentation and compliance with medical‑aid requirements.

Key responsibilities

• Develop and manage individualized case‑management plans in collaboration with a multidisciplinary team.
• Conduct initial and ongoing clinical assessments and monitor patient progress.
• Liaise with medical‑aid providers, funders and external healthcare services to obtain and extend treatment authorisations.
• Prepare detailed clinical motivations, progress reports and apply accurate ICD‑10 diagnostic codes.
• Facilitate communication between patients, families, healthcare professionals and stakeholders.
• Coordinate referrals, transfers, follow‑up appointments and after‑care arrangements.
• Identify and escalate clinical, psychosocial and operational risks while maintaining confidentiality.

Required profile

• Registered Nurse qualification with current SANC registration.
• Proven case‑management experience in a healthcare setting.
• Strong knowledge of ICD‑10 coding and medical‑aid authorisation processes.
• Experience in private healthcare, psychiatric, mental health, addiction or rehabilitation settings is advantageous.
• Proficiency in Microsoft Office and electronic clinical‑record systems.
