Clinical Nurse Coordinator

About the role

The Pain Collective is seeking a Clinical Nurse Coordinator to join its central clinical support team. This full‑time position is based at the organisation’s central hub and can be performed in a hybrid or remote setting. The role focuses on coordinating patient journeys, providing telephonic clinical follow‑up, and acting as a liaison between patients, doctors and administrative staff.

Key responsibilities

• Coordinate patient pathways across multiple practitioners and clinics, ensuring seamless communication.
• Serve as the primary clinical point of contact for patients and families, delivering education on procedures, recovery expectations and treatment plans.
• Conduct structured follow‑up calls after interventional pain procedures, monitor progress, document outcomes and flag any complications.
• Support treating doctors with patient communication, care coordination and outcome monitoring.
• Provide clinical guidance to administrative teams and assist with obtaining necessary documentation.
• Participate in clinical audits, quality‑improvement initiatives and ensure compliance with relevant regulations.

Required profile

• Registered Nurse with experience in patient engagement and care coordination.
• Highly organised, able to work independently and manage multiple interactions simultaneously.
• Strong communication skills and a compassionate, professional approach.
• Comfortable using telephone and digital tools for patient follow‑up.
